In order to insure your having a CATHOLIC BURIAL, with full rites and privileges of the Church, it is most important to leave clear and definite directions to those who will be responsible for your funeral.

This should be done in consultation with your parish priest.

The parish provides a form to use, which you can complete and mail to the Parish Administrator and to members of your family or to whomever will be responsible for your funeral.
